Telling me a bit about yourself & giving me permission to review your medical records online.

I check that I can access your medical records
I will respond to you after I am able to access your records or advise you how to obtain your own records.

When record access is sorted, you make an appointment, submit a few more details & pay a fee online.
We discuss your medical history face-to-face or via a video call.

I provide a medical for you to use in your application
You attach the medical report to the relevant part of Form 201, your permit application.
About Dr Chambers
Dr Mungo Chambers qualified from Bristol Medical School in 2002. He became a member of the Royal College of General Practitioners in 2007. He has been working as a full-time GP partner in Gloucestershire since 2013.
He is a keen country sports enthusiast with a particular passion for shooting. He is dedicated to the safe provision of licenses for all types of firearms.
